Introduction

Psoriasis is a chronic, immune-mediated inflammatory skin disorder characterized by the accelerated proliferation of epidermal cells. This process results in the formation of erythematous plaques covered with silvery-white scales, commonly affecting the elbows, knees, scalp, and extremities. The condition is often accompanied by itching, dryness, scaling, excoriation, and, in severe or complicated lesions, skin fissuring and oozing.

Ozone therapy has been explored as a supportive approach in various dermatological conditions due to its proposed therapeutic mechanisms, which include the modulation of oxidative stress, influence on inflammatory processes, and support for tissue repair. Topical ozone bagging is a specific method of administration in which an affected body part is enclosed in an ozone-resistant bag to allow for controlled exposure to medical ozone.

Case Presentation

A 29-year-old female patient presented with chronic skin lesions over both upper limbs that had persisted for the past six years. She reported continuous skin changes associated with scaling, itching, excoriation, and occasional fluid oozing from the affected areas, alongside noticeable skin inflammation and surface changes.

Summary of Symptoms:

  • Chronic skin lesions across both upper limbs for six years
  • Persistent itching over the affected areas
  • Prominent scaling and dryness of the skin
  • Excoriation resulting from repeated scratching
  • Oozing of fluid from the affected skin lesions
  • Persistent skin irritation and discomfort

Clinical Examination

An integumentary system examination yielded the following findings:

  1. Inspection:
  • Skin Colour: Altered skin colour over the affected regions, associated with chronic inflammatory skin lesions.
  • Distribution: Lesions localized to both upper limbs.
  • Pattern and Configuration: Psoriatic skin lesions involving the targeted areas.
  • Surface Characteristics: Prominent scaling, excoriation, and areas of fluid oozing.
  • Skin Integrity: Disruption of the superficial skin layer over excoriated zones.
  • Inflammatory Changes: Visible signs of active skin inflammation.
  1. Palpation:
  • Skin texture and surface characteristics were evaluated over the affected regions.
  • Local temperature and tenderness were assessed.
  • Skin thickness and consistency were evaluated where clinically appropriate.

Clinical Impression: The findings were consistent with chronic psoriasis involving both upper limbs.

Therapeutic Intervention

As part of her clinical management, the patient underwent topical ozone bagging therapy. The procedure involved enclosing the affected upper limbs in an ozone-resistant bag and administering medical ozone under controlled clinical conditions. A total of six sittings of topical ozone bagging therapy were administered, accompanied by appropriate monitoring throughout the treatment period.

Clinical Outcomes

Following the completion of six sittings of ozone bagging therapy, notable clinical improvements were observed in the affected areas. These improvements included:

  • A reduction in visible scaling
  • A decrease in excoriation
  • A reduction in fluid oozing from the lesions
  • Improvement in the overall appearance of the affected skin
  • Early signs of skin healing and recovery

These changes indicate a favourable short-term clinical observation following the treatment sessions.

Conclusion

This case highlights the clinical improvement observed in a 29-year-old female patient with a six-year history of psoriasis after receiving six sittings of topical ozone bagging therapy. The reduction in scaling, excoriation, and fluid oozing, combined with early signs of skin healing, represents an encouraging clinical observation. Further clinical assessment and long-term follow-up are necessary to evaluate the sustainability of these changes and to establish the exact role of topical ozone therapy in psoriasis management.
