Managing Complex Musculoskeletal Pain and Lifestyle Conditions Through Integrative Naturopathy: A Clinical Case Report
As a practitioner specializing in Yoga and Naturopathy (BNYS), with a focus on diet and nutrition, diet therapy, integrative clinical practice, integrative pain medicine, and lifestyle disease management, I frequently encounter patients presenting with multi-system health concerns. A recent case at Shri Sivalaya Holistic Natural Healing Hospital in Kangayam demonstrates the effectiveness of an individualized, integrative naturopathy and rehabilitation programme in managing complex musculoskeletal pain alongside chronic lifestyle conditions.
Patient Presentation and Clinical History
A 54-year-old female presented with complaints of left knee pain—particularly during standing and walking—lasting 3 to 4 months, and right shoulder pain with a restricted range of motion for approximately 6 to 7 months. She also reported a history of hard stools and constipation for about one year.
Her significant medical history included retinal derangement surgery, bronchial asthma, systemic hypertension, and dyslipidemia.
Upon admission, her vital signs and physical assessments revealed:
- Appearance: Obese
- Height: 150 cm
- Weight: 93.6 kg
- Blood Pressure: 145/95 mmHg
- Bowel Function: Irregular (all other vitals were normal)
Clinical Examination and Investigations
A thorough locomotor examination yielded the following findings:
- Knee: Left knee showed iliotibial (IT) band stiffness and a restricted range of movement.
- Shoulder: Right shoulder demonstrated restricted movement, specifically in lateral rotation.
An MRI of the knee revealed mild to moderate osteoarthritis with a grade 3 chondromalacia pattern, a complete tear in the posterior horn of the medial meniscus, and mild joint effusion.
Based on our clinical assessment and investigations, the patient was formally diagnosed with:
- Degenerative joint disease (DJD) of the knee
- IT band stiffness
- Right-side periarthritic shoulder
- Constipation
- Obesity
- Systemic hypertension
- Acid peptic disease
- Impaired glucose tolerance
- Vitamin D deficiency (noted in medical records)
Treatment Plan
To address her symptoms and improve functional mobility, the patient was admitted for a structured, 9-day integrative naturopathy and rehabilitation programme tailored to her specific needs.
The treatment plan encompassed:
- Hydrotherapy and Mud Therapy: Kidney pack, castor oil pack, full mud bath, and plantain leaf bath.
- Soft Tissue Therapies: Full body massage, kizhi, and localized massages for various body parts.
- Physiotherapy Modalities: Wax therapy, interferential therapy (IFT), and suspension therapy.
- Therapeutic Exercises: Resistance exercises, straight leg raise (SLR) exercises, and prone SLR exercises.
- Diet Therapy: Juice fasting utilizing ash gourd juice, watermelon, papaya, muskmelon, and bottle gourd juice.
Results and Discussion
Following the 9-day integrative naturopathy treatment programme, the patient demonstrated notable clinical improvements:
- Reduction in pain over both the knees and the shoulder.
- Significant reduction in blood pressure, dropping from 145/95 mmHg to 112/84 mmHg.
- Weight reduction from 93.6 kg to 90.3 kg.
- Regularization of bowel movements.
Overall, the patient’s positive progress highlights the efficacy and importance of individualized care in managing musculoskeletal pain and associated health concerns through naturopathy. Regular assessment and appropriate follow-up remain essential to evaluate the long-term sustainability of these observed improvements.
